This project works best for students who have a personal health goal - like quitting smoking, losing weight, improving eating habits, starting an exercise program, etc.
Essentially you will be explaining your goal, gathering research to help you meet your goal, journaling as you try to reach your goal and evaluating how you did in reading your goal.
This will be typed and all on one page. Be sure it has a proper heading.
Create an Annotated Works Cited for your goal/topic. This will be typed and include at least 5 reliable sources. After locating a resource that will support and help you in achieving your goal, create a Works Cited entry for it (proper and full format). Then you type a paragraph below that entry that explains how you plan to use that resource, why it is a good match for your topic and how you know it is a reliable source. (This is professional-sounding writing...defend why this resource is the right one to use.)
Keep a daily log for the entire month (start when you get Step One signed by teacher). Each entry should be dated and be at least one paragraph. In the paragraph explain who you did in meeting your goal that day. Mention any challenges and how you handled them. Celebrate your successes and regroup if you slide a little. be sure to mention when and how you are using information from your Works Cited. Show how that research fits into your plan to reach your goal. Your teacher will ask to see this as you go - so don't plan to fake it at the end of the month. This writing is informal, but should still respect basic grammar, spelling and punctuation rules.
Write at least one page (double-spaced) that reviews your goal, your plan and then evaluates how you did for the one month.

This high-quality project makes it easy for the reader to understand what, why and how the student did. The writing has obviously been proofread and edited. Each step is complete and as a whole they need no further explanation. |

This project has the info required in all four steps. The writing is not A quality or any one of the steps might be lacking in quality or polish. There are no glaring typos/errors that distract the reader. The reader can tell what the goal was and how the student did for the month in reaching that goal. Some verbal questioning might be necessary to really understand how things went. |
